Billing and Insurance
Our practice accepts all the major private insurances and Medicare. Our practice requires that all patients keep a valid credit card on file. In the event that a patient has a co-insurance, deductible, a change in co-pay amount or your visit is not covered due to a pre-existing condition or for any other reason your card will be charged.
We use electronic medical records and the database requires us to scan a copy of the driver license. We request that you provide us with that at the time of initial visit. An email address is also requested to allow the physician to communicate with you regarding labs and other health matters. We’re committed to your privacy and we will only use your e-mail for communicating with you and not for any other purpose. Review our complete confidentiality policy.
